PhysioElite is a multidisciplinary practice based in Templetuohy, Tipperary.
In order for you to receive the most efficient treatment possible, it is essential that we gather some information from you. This information will be stored and secured under current GDPR guidelines.
This information will be used to contact you regarding appointments, exercise or homecare programmes, referrals, invoicing and for any other reason to improve the service we can provide to you.
If we feel referral is best for you and your particular condition, we may need to share this information with other practicing therapists/GP’s/consultants/other health professionals. This will only be done once your full consent has been gathered in the first instance. It is unlikely that this information will be transferred outside of the EU but if this is deemed necessary, in consultation with you, and with your prior consent, it is a possibility.
Obtaining some of the information that will be gathered through your assessment is a legal requirement for a practicing therapist. This information will be retained for a period of 5 years (or for a period of 5 years after the age of 18 is reached) in a secure manner. You have the right of complaint where you are unhappy with your implementation of any of these criteria. Your rights as an individual can be viewed on gdprandyou.ie
